The pipeline business now has many years of operational experience. This, combined with access to sophisticated inspection tools such as intelligent pigs, improved coatings and protection, and advanced calculation methods for the assessment of defects, should ensure that pipelines are both safe and secure. However, there continues to be frequent and tragic pipeline failures around the world. What lessons can we learn from these inspections, assessments and failures? This paper considers some of the lessons learnt from recent failures and field experiences, and concludes with some guidance for all engineers in the pipeline business.
