Abstract

This paper introduces a highly versatile domino evaluation system-Hierarchical Analyst Domino Evaluation System (HADES)-incorporating the most advanced industrial risk assessment system for Domino Effect Accidents (DEAs). HADES differs from traditional DEAs risk assessment methods that use empirical formulas extensively to calculate the probability of risk or damage. Instead, it takes the DEAs triggering mechanisms as the core and the industrial process as the backbone, upgrades the QRA framework with AHP idea, evaluates the accidents’ physical effects, and compares the target criteria database for forming accident sequence dynamically. The decommissioning of an offshore oil and gas facility in the North Sea was used as a case study to show how HADES works. In general, with approximately modifications, HADES can be widely used in many industries with little adaptation and modification.
