Accuracy of Dexcom G7 and Freestyle Libre 2 during acute exposure to hot, dry conditions in healthy adults

Abstract

Aims: Increasing hot weather (≥28°C) in the UK may impact the clinical accuracy of continuous glucose monitors(CGMs). This seminal study investigated accuracy of two CGMs in thermoneutral (TN) and hot (HOT) environments during a simulated meal and light exercise. Methods: Nine healthy adults (male = 4) made two 3h lab-oratory visits under TN (21°C, 40% RH) and HOT (35°C,40% RH) conditions. Dexcom G7 (G7) and Freestyle Libre2 (L2) CGMs were applied ≥24h prior to the first visit.Participants completed a 90min oral glucose tolerance test(OGTT) followed by a 30min walk (EX). CGM values were compared with capillary blood glucose (CBG) (Biosen C-Line) and mean absolute relative difference (MARD) was calculated. Data were analysed using a repeated measures ANOVA with Bonferroni post hoc. Results: During the OGTT, MARD significantly improved in HOT vs TN for L2 (15.45 vs 21.43%; p = 0.030)but not G7 (15.25 vs 17.02%; p = 0.412). No difference was found between HOT and TN during EX (G7: 14.66vs 20.97%; L2: 10.12 vs 16.44%; p ≥ 0.195). Across both conditions, G7 reported significantly higher glucose than CBG during OGTT (7.36 vs 6.50 mmol L−1) and EX (5.31vs 4.58 mmol L−1; p ≤ 0.001). L2 reported significantly higher glucose than CBG in both conditions (7.40 vs6.50 mmol L−1) during OGTT (p ≤ 0.001) only. Conclusions: Contrary to our hypothesis, preliminary data suggest acute heat exposure may improve CGM accuracy following a simulated meal compared with TN conditions; however, currently this is neither consistent across sensors or during light exercise.
