Objective: To conduct the cross-cultural adaptation and psychometric validation of the OHIP-TMDs for Portuguese-speaking populations, developing a unified version that encompasses both European and Brazilian Portuguese variants. Methods: A bicentric observational study was performed, following Beaton’s guidelines and COSMIN standards. The OHIP-TMDs was translated and culturally adapted through a rigorous process including expert review and pre-testing. Psychometric validation was carried out with 211 participants (98 from Portugal, 113 from Brazil) diagnosed with TMD. Internal consistency (Cronbach’s α, McDonald’s ω), test–retest reliability (ICC), structural validity (CFA), convergent validity (via Fonseca Anamnestic Index), and known-groups validity were assessed separately in both countries. Results: The OHIP-TMDs-PT showed excellent internal consistency (α = 0.98 PT; α = 0.96 BR) and test–retest reliability (ICC = 0.96 PT; ICC = 0.94 BR). CFA supported a unidimensional model in Portugal and a second-order hierarchical model in Brazil. Strong convergent validity was confirmed through correlations with FAI (ρ = 0.72 PT; ρ = 0.66 BR). Known-groups comparisons revealed significant differences across TMD severity levels and between clinical and non-clinical groups. Conclusion: The OHIP-TMDs-PT is a culturally adapted, psychometrically robust tool for assessing oral health-related quality of life in individuals with TMD. The development of a unified version strengthens its applicability across Lusophone populations, supporting clinical evaluation and cross-national research collaboration.
