Autoimmune diseases are increasingly recognised as aetiologies of bronchiectasis, and overlap syndromes are associated with reduced autoimmune disease control and poorer outcome in terms of infection and mortality rates than either condition alone. It is therefore important to recognise coexisting autoimmune diseases in bronchiectasis and, when necessary, to perform additional tests and refer to rheumatologists. Once a diagnosis is established, management changes are warranted beyond usual bronchiectasis care, with particular emphasis on lowering thresholds for earlier treatment across physiotherapy, mucoactive therapy, antibiotics, inhaled agents, immunomodulation and specialist referral. Furthermore, multidisciplinary care with coordinated pulmonary–rheumatology management is essential to maintain a balance between autoimmune disease control and respiratory infection risk.
