If clinical governance is to be integrated into clinical practice and provide a system for developing and monitoring quality of care, policy-makers and managers need to understand the culture in which the medical profession is educated, trained and practises and how this shapes its thinking about managerially driven policy. With such an understanding, health policy-makers and managers (including doctors) could implement strategies that are sympathetic to the very powerful values of the profession and facilitate clinician's engagement with clinical governance. This chapter outlines the factors that shape a doctor's practice and discusses what doctors, managers and medical training can do to embed clinical governance.
